On the 23rd of June, 1985 Air India flight 182 was blown up over the Atlantic Ocean by a bomb placed on board by Khalistani Terrorists. These terrorists operated out of Canada and wanted a separate Sikh homeland. Their fight for power within the Global Sikh establishment led them to kill innocent Hindu's and even Sikhs under the pretence of a being protestors of the Sikh faith. The flight disintegrated mid-air and killed all 329 passengers, made up of 24 Indian nationals, 268 Canadians and 27 British. However the passengers were predominantly Hindu's and Sikh that had migrated to those countries earlier. It came to be the Largest Mass Killing in Canadian History.
Amongst the dead were up to 86 children and 29 entire families. Other children were orphaned as a result of the event. The trial went on for 22 years and cost the Canadian taxpayer $130 million. In the end only one person was actually convicted Inderjit Singh Reyat, a Canadian national and a member of the IYSF. He only recieved a sentence of 15 years for having made the bombs that exploded on Flight 182 and at Narita Airport. A subsequent report did conclude that the Canadian Government, Canadian Security Forces and the Royal Mounted Police had allowed the attack to take place given their grave errors.

Inderjit Singh Reyat, Convicted Terrorist Bomb Maker
The investigation was hampered throughout and the case actually brought about changes such as reducing the ability for terrorist to be funded, how witness protection in terrorist cases is provided and upgrades in airport security. You can see why with some of the issues that hampered the case below:
Canada did however cover the event through memorials, books, articles as well as Ireland. But the investigation clearly indicates that this entire catastrophe could have been avoided.
Investigators believed that there was a second plot planned by the same terrorists, but this time the bomb went off at New Tokyo International Airport (Later renamed: Narita Airport in Japan. The bomb luckily didn't go if in the air, but it still killed two baggage handlers and injured a further 4 people. The bomb was intended for another Air India Plane, flight 301 with 177 people on board for Bangkok. Had the bomb not had blown up while the luggage it was in was being transferred, it would have resulted in the loss of many more lives.
